Canthoplasty vs. Canthopexy: What’s the Difference?

Quick answer: Both reshape the outer corner of the eye, but they differ in how much they do. A canthopexy tightens and repositions the existing outer corner tendon without cutting it — a subtler lift. A canthoplasty detaches and reconstructs that corner, allowing a bigger change. The right one depends on your eyelid laxity and goals.

Canthopexy: the tightening

A canthopexy reinforces and slightly repositions the outer corner (lateral canthus) without releasing the tendon. It’s the more conservative option — good for a modest lift or to support the lower lid — with generally quicker recovery.

Canthoplasty: the reconstruction

A canthoplasty actually detaches and remakes the outer corner, so it can create a larger change in shape or correct significant lid laxity or malposition. More is done, so it’s a bigger procedure.

How the right one is chosen

The decision rests on your eyelid tone, corner position, whether there’s a functional issue, and how much change you want. Sometimes these accompany a blepharoplasty or a fox-eye request. An exam decides.

Natural results either way

With both, the goal is a natural, supported outer corner — not an obviously pulled eye. Overcorrection is the main way these look unnatural.
